Job Overview
WE CARE HOME CARE LLC is seeking a passionate and organized Designated Coordinator / Qualified Professional (QP) to join our team in Mankato, MN. This full-time, on-site position ensures compliance with MN Statute 245D and CFSS program standards, oversees service delivery, supports staff, and maintains accurate documentation.

The ideal candidate is detail-oriented, compassionate, and knowledgeable about Minnesota DHS regulations and person-centered practices.

Duties

• Develop, implement, and review Individualized Service Plans (ISPs) and CFSS care plans.
• Conduct regular quality assurance audits and service reviews.
• Supervise and support Direct Support Professionals (DSPs).
• Maintain compliance documentation, progress notes, and incident reports.
• Communicate effectively with case managers, guardians, and interdisciplinary teams.
• Participate in licensing reviews, staff training, and quality improvement initiatives.
• Promote person-centered, respectful, and compliant service delivery.

Qualifications

• Meets Minnesota Department of Human Services (DHS) qualifications for Designated Coordinator or Qualified Professional (QP) under MN Statute 245D.

Preferred:

• Bachelor’s degree in Human Services, Social Work, Nursing, Psychology, or a related field.
• 1–2 years of experience coordinating or providing services to individuals with disabilities or vulnerable adults.
• Experience using electronic health records (EHR) or case management systems.
• Knowledge of 245D and CFSS program standards and compliance practices.
• Strong organizational, communication, and leadership skills.
• Attention to detail and ability to manage multiple priorities.

Job Type: Full-time

Pay: $50,000.00 - $65,000.00 per year
