Description

Summary

The Donor Family Aftercare Coordinator (AC) advances the mission of We Are Sharing Hope SC (SHSC) by providing compassionate, ongoing support to donor families following their loved one’s gift of donation. This role ensures donor families remain connected, honored, and supported through meaningful programs, individualized follow-up, and opportunities to celebrate the legacy of donor heroes.

In collaboration with the Family Services leadership team, the Aftercare Coordinator develops, implements, and sustains aftercare initiatives that align with SHSC’s values, integrate best practices in grief support, and strengthen relationships with donor families, community partners, and transplant centers. This position plays a vital role in demonstrating SHSC’s enduring commitment to donor families and ensuring every legacy is honored with dignity and care.


Essential Duties & Responsibilities

Family Support & Advocacy

  • Provide compassionate support to donor families through ongoing outreach, including regular phone calls, written communication, and in-person meetings.
  • Respond to donor family inquiries and requests in a timely and empathetic manner.
  • Support families through the written correspondence process with transplant recipients; serve as liaison to transplant centers to facilitate connections.
  • Coordinate donor family–recipient meetings in accordance with SHSC standards.
  • Act as a resource and advocate for donor families with funeral homes, coroners, and community partners when needed.

Aftercare Programs & Engagement

  • Plan, coordinate, and execute aftercare engagement opportunities for donor families, including remembrance ceremonies, grief workshops, family events, and special recognition programs.
  • Develop and manage the Donor Family Council, including recruitment, structure, meeting facilitation, and follow-up on initiatives.
  • Create and sustain opportunities for donor families to participate in national events, such as the Transplant Games of America and regional events, such as hospital rounding or conferences.
  • Serve as liaison for donor families interested in community engagement and volunteerism with SHSC and donation partners.
  • Incorporate family feedback into aftercare programs, ensuring they remain responsive, meaningful, and supportive.

Documentation & Program Evaluation

  • Accurately document and trend family interactions, calls, letters, and engagement activities in the electronic medical record according to SHSC standards.
  • Develop, distribute, and analyze surveys to donor families to measure the effectiveness of aftercare services.
  • Track and report on aftercare activities, family participation, and outcomes to inform program growth and continuous improvement.
  • Maintain current knowledge of community grief resources and sustain a comprehensive library of support materials.

Collaboration & Leadership Support

  • Partner with the Director and Manager of Family Services to align aftercare services with departmental and organizational goals.
  • Assist with the training of SHSC staff regarding aftercare processes and donor family engagement.
  • Contribute to departmental and organizational policy and procedure updates related to aftercare.
  • Actively participate in meetings, special projects, and community partnerships as assigned.

Other Duties

  • Perform additional responsibilities in support of SHSC’s mission, as assigned.

Qualifications


  • Bachelor’s degree in Social Work, Psychology, Counseling, or related field preferred.
  • Minimum 2 years of experience supporting individuals through grief, bereavement, or end-of-life care required.
  • Grief counseling certification preferred.
  • Experience working with families in sensitive or high-stakes situations strongly preferred.
  • Knowledge of organ, eye, and tissue donation processes preferred.
  • Strong communication, active listening, and interpersonal skills, with the ability to demonstrate empathy while maintaining professional boundaries.
  • Organizational skills with ability to manage multiple projects, events, and family needs simultaneously.
  • Equivalent combination of education and experience may be considered.
