Staff Nurse (LPN) - Day Habilitation Program - Haverhill, MA
Waystone is currently seeking a dedicated LPN to provide medical care for Individuals with intellectual and developmental disabilities. As our Day Program LPN, you will support the wellbeing of individuals in our Day Habilitation programs by assisting with clinical assessments, administering medications, providing treatments, and on personal care.
Location: Haverhill, MA (Day Habilitation Program)
Part Time Schedules 25 hours per week
This position is benefit eligible as of date of hire.
- Monday - Friday 8:30am - 1:30pm.
Compensation range: This is a 25-hour non-exempt, part-time position. The hourly pay range is $32.00 - $38.30.

Job Responsibilities:
- Deliver skilled nursing care, including clinical assessments, treatments, and medication administration.
- Collaborate with individuals, families, case managers, and program staff to ensure continuity of care.
- Monitor and document health histories, vitals, medications, and treatments.
- Maintain accurate medical records in compliance with agency policies and regulatory guidelines (DMA, DDS, DPH, CARF).
- Support program activities by assisting with personal care, dietary needs, tube feedings, and ADLs.
- Accompany individuals on community outings and provide medical oversight as needed.
- Foster a respectful, inclusive environment that upholds the rights and dignity of every individual.
Qualifications:
- Current Massachusetts LPN license in good standing
- Experience working with people with intellectual and developmental disabilities is a plus.
- Candidates must be able to read, write, and comprehend English to accurately follow care plans, document services provided, and communicate effectively with individuals, families, and healthcare professionals.
- Organizational skills, computer literacy and the ability to document electronically.
- The ability to travel within the service area of residential programs and medical appointments.
- Valid driver’s license in good standing and a minimum of one year of driving experience; must be able to drive wheelchair-accessible vans.
