Youth and Family Support Worker (Full-Time)
Milford, MA
Duties:
Provide community-based, trauma-informed support services to children, youth, and families referred by agencies such as the Department of Children and Families (DCF), Department of Mental Health (DMH), or through insurance-based referrals. Deliver direct, in-home services focused on behavioral health, family functioning, and social service needs of youth and families. Utilize evidence-informed interventions to promote skill-building in areas such as emotional regulation, social functioning, daily living skills, and symptom management. Coordinate care plans with families and multidisciplinary teams including clinical staff, schools, caseworkers, and community providers. Participate in crisis intervention as appropriate, helping families de-escalate situations and connect to emergency supports. Provide both therapeutic (soft) services (e.g., mentoring, behavior coaching, coping skills development) and concrete (hard) services such as transportation, referrals to food, housing, or furniture resources. Attend and advocate at care planning meetings, school meetings, DMH or DCF team meetings, and other collateral appointments as needed. Maintain accurate and timely documentation in electronic health records for all client contacts, care coordination efforts, and progress updates. Collaborate regularly with referral sources, agency partners, and other treatment team members to ensure service alignment and continuity of care. Manage a caseload to meet agency productivity requirements and program expectations. Deliver services in a manner that is culturally responsive and respectful of the individual needs of diverse client populations.
Requirements:
Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or related behavioral health field (or foreign equivalent) and 2 years of experience working with children, young adults, and/or families in a behavioral health, educational, or community services setting. At least 2 years of experience navigating public benefits programs and community mental health systems; At least 2 years of experience interacting with individuals with mental health or behavioral health challenges; At least 2 years of experience organizing and advocating for access to community-based supports such as after-school programs, food assistance, daycare, or mental health services; At least 2 years of experience with trauma-informed care models including Motivational Interviewing, Courageous Conversations, and the Attachment, Regulation, and Competency (ARC) framework.
25% travel required throughout MA to deliver in-home, community-based services to client and attend external meetings (e.g., school, DCF, DMH, planning meetings) as needed.
Salary: $51,106/Year- $58,240/Year
