In-Home Clinician (IHT, FIT, ICC, DCF, DMH)
Community Based - Framingham
Salary Range: $65,000.00 - $70,000.00/year sign-on bonus (half paid after 6 months of employment and half paid after 1 year of employment/pro-rated for part-time).

What you will do:
- Conduct individual and /or family counseling using trauma-informed and culturally responsive interventions for youth and their families.
- Provide intensive family-focused therapeutic services in home, school, and community settings to support youth with significant behavioral health needs.
- Complete comprehensive behavioral health assessments, develop individualized treatment plans, and monitor progress toward measurable treatment goals.
- Assist caregivers in navigating child-serving systems, promoting awareness of resources and improving access to supports and services.
- Empower caregivers with skills and strategies to support their child's behavioral health needs and strengthen family functioning.
- Create care plans and interventions to support skill-based growth, family stability, and long-term success in the community.
- Promote and strengthen families' natural supports and community connections to foster sustainable treatment gains and positive outcomes.
- Provide crisis prevention, risk assessment, safety planning, and stabilization interventions to support youth and families during periods of increased need.
- Work as part of a multidisciplinary team with the child, parents/caregivers, Family Partners, schools, medical providers, and other service providers to ensure close collaboration and coordination of care.
- Facilitate ongoing care coordination through phone, virtual, and in-person meetings with providers, schools, state agencies, and community partners to support continuity of care and treatment planning.
- Coordinate referrals, community linkages, and warm handoffs to behavioral health, medical, educational, and community-based services to ensure successful transitions and continuity of care.
- Support discharge planning from the beginning of treatment, including connecting youth and families to ongoing services and developing aftercare plans that promote continued success.
- Collaborate with community-based organizations and service systems to advocate for youth and families and ensure access to needed supports.
- Ensure that clients' rights are respected, particularly regarding cultural responsiveness, family voice and choice, and equitable access to services.
- Talent connecting with people.
- Strong communication skills – both written and verbal.
- Ability to assess and manage risk.
- Strong care coordination, collaboration, and systems-navigation skills.
- Ability to engage youth and families experiencing complex behavioral health challenges.
- Flexibility, creative thinking, and ability to adapt to competing priorities.
- Master's Degree in Social Work, Mental Health Counseling, or related field.
- License eligible required; independent licensure (LICSW, LMHC, LMFT, LCSW) preferred.
- Experience providing family therapy, community-based behavioral health services, crisis intervention, or care coordination strongly preferred.
- Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.
