Our team is growing! Waypoint (formerly know as The Progress Center) is seeking a skilled, motivated, and highly organized professional to join our growing Children's Behavioral Health Home program.

Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.
About the Role:
The Care Coordinator supports children and adolescents from birth to 21 years of age who have developmental disabilities and/or behavioral health needs by working with the child's family/caregivers to assess and identify needs, develop an individualized plan of care, and organize services and supports. The Care Coordinator is a member of the Children's Care Management Team that also includes Nursing, Psychiatry, Medical, and Family Supports providers.

Requirements
Qualifications:
- BA/BS in human services or closely related field or
- BA/BS in unrelated field minimum 1 year experience in relevant children's services
- Knowledge of the system of care in Maine preferred
- Reliable vehicle with proper insurance
- Valid driver's license issued by state of residence
- Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring
