Minimum Qualifications
A. Graduation from a regionally accredited or NY State registered four-year college or university with a bachelor’s degree in a health or human services-related field (e.g. nursing, psychology, sociology, gerontology, social work, health science); or
B. Graduation from a regionally accredited or NY State registered two-year college with an associate degree in a health or human services-related field and two years of full-time paid experience in human services (e.g. nursing, community health, social services); or
C. Graduation from high school and four years of full-time paid experience in human services (e.g. nursing, community health, social services).
Typical Work Activities
- Assists caseworkers in carrying out service plans for clients;
- Make collateral contacts on open services cases or investigations and documents;
- Perform/Assist with determining services program eligibility and ongoing eligibility, route referrals and authorizing services and payments including case openings, case maintenance, recertification, foster care movements, functions related to coding cases for services and billing, representative/protective payee, and enrolling eligible families into appropriate programs according to Federal, State and local social service regulations;
- Act as a liaison between clients and other units of the Department of Social Services to ensure that benefits and services have been applied for and authorizing paperwork is processed;
- Consult with both families and providers in person, by phone and by letter;
- Prepare progress-notes and other documentation related to services or contacts with clients and families;
- Request and assist child welfare staff in review of various client paperwork submitted to the agency related to case planning, eligibility determination or other program activities;
- Transport clients (youth and families) and deliver paperwork or goods as needed;
- Perform home visits for the purpose of obtaining information relevant to casework;
- Attend case related meetings including Service Plan Reviews;
- Perform basic office and clerical duties including typing, creation of case files with supporting documentation, answering phones, faxing records requests, and filing;
- May receive reports of Child Abuse or neglect to perform searches pertaining to case histories, conduct person identification search, absent parent search, relationship matrix, and case merges to avoid duplicate reporting.
