Job Overview
Under the general direction of the Director of Nursing, the Creating Healthy Communities (CHC) Coordinator plans, develops, and implements population health promotion objectives/activities and general health education activities in specific health areas determined annually by the agency and program grants (s). Acts as a public health education resource to community agencies and the general public. Assists as needed with the agency’s media/social media presence. Works with the Director of Nursing and Director of Finance to ensure compliance with applicable program standards, grant requirements, state regulations, and Board of Health adopted policies. This position is supported by grant funds.
Duties
75%Prepares grant applications and monthly reports for Creating Health Communities. Represents Agency at state and local public health education-related meetings. Attends trainings (including train the trainer) to effectively administer public health education initiatives. Evaluates education programs, tracks outcomes, and recommends changes using data analysis.
15%Coordinates and implements public health education programs, including grants and community collaborative projects to promote public health outcomes.
5%Responds to public health emergencies as appropriate. Contribute to continuous quality improvement of program and organizational performance, and perform all other duties as assigned or directed to promote the Wayne County Health Department.
5%Update and keep social media platforms current. Market health department programs and services in a variety of venues.

- Collect and analyze data related to community health outcomes and program effectiveness
- Conduct program needs assessment and prepares annual grant proposal and work plan, naming goals and objectives for specific population health program activities in order to meet identified community
- Engage with community members to understand their health concerns and preferences, organize and facilitate community meetings, workshops, and events related to creating healthy community projects, foster partnerships with local organizations and businesses to support community health initiatives

Pay: $18.00 - $24.00 per hour
