Essential Duties and Responsibilities
Position Summary
As a service coordinator, your role is to manage and provide access to necessary supportive services in the community, provide case management services as needed and requested, and develop programs and resources that support wellness for the entire resident population.
Essential Duties and Responsibilities
· Advocates on behalf of their residents
· Perform wellness checks on a weekly basis
· Motivators who empower residents to be as independent as possible
· Referral agents who connect residents to service providers who can meet their needs.
· Assist clients with applications for benefits and entitlement programs.
· Answer telephones and respond to email inquiries
· Prepare monthly reports for supervisors
· Maintain accurate records of service requests
· Collaborate with RAIN and Wavecrest departments to ensure smooth service delivery
· Seek for programs and activities available to seniors
Required Knowledge, Skills, and Abilities
Minimum Qualifications
· Strong work ethic and service skills
· Excellent communication and interpersonal skills, Bilingual/Spanish
· Strong record-keeping and analytical skills
· Excellent organizational skills and attention to detail
· Basic computer skills (Outlook, Excel, Microsoft Word) and all computer-based documents management programs.
