What are the responsibilities and job description for the Comprehensive Community Services (CCS) Facilitator position at Waupaca County?
As a function of the Waupaca County Human Services Department, the CCS Facilitator uses a trauma informed, teaming, or wrap-around approach to provide direct case management and service facilitation to adults and children with significant mental health, substance use, or co-occurring issues.
This position works under the direct supervision of the Behavioral Health Manager of Wraparound Treatment Services.
Provide case management and service facilitation for both adults and youth.
Create a consumer centered team of professionals and natural supports that wrap around the consumer to support their recovery and/or rehabilitation goals.
Complete assessments and a functional screen. Develop and create individual recovery plans.
Develop crisis plans and crisis intervention services as required.
Complete and comply with all documentation and program billing requirements.
Participate in monthly supportive and clinical supervision at minimum of one hour per week or every 30 hours of face-to-face contact with consumers.
Carry out the mission and vision of DHS to help the people of Waupaca County to be safe and connected.
Able to work up to three evenings per week to accommodate consumers’ school and work schedules.
PERIPHERAL RESPONSIBILITIES:
Create working relationships with primary care providers, psychiatrists, Coordinated Services Teams, family services, school counselors, other professionals, and natural supports.
Attend staff meetings and trainings as directed.
Provide services in an approach that incorporates Trauma Informed Care principles.
Performs other duties as assigned.
EDUCATION: Bachelor’s degree from an accredited college in social work, psychology, counseling, or related field. Social work or other licensure preferred.
EXPERIENCE: Experience working with both youth and adult individuals and/or families who have mental health and SUD(substance use disorder) diagnoses. Knowledge and understanding of recovery based treatment. Case management experience preferred.
SPECIAL REQUIREMENTS:
CCS orientation training upon hire. 8 hours of CCS training annually thereafter.
Knowledge of Trauma Informed practice.
Must possess basic knowledge of mental health and SUD services. Have an understanding of state chapter 36 and common state programs available to clients.
Ability to maintain accurate and detailed records.
Ability to communicate effectively orally and in writing.
Ability to establish and maintain effective working relationships.
Ability to maintain appropriate confidentiality.
Must have available transportation to perform on-site and off-site responsibilities. Use of county vehicles is available at times.
