LPN Staff Nurse

Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans for residents, in collaboration with the interdisciplinary care team
Maintain accurate and detailed records of patient care, including progress notes, medication administration records, and care plans
Collaborate with other healthcare professionals to ensure seamless coordination of care and optimal patient outcomes
Participate in quality improvement initiatives and staff education programs to improve patient care and promote best practices
Demonstrate a commitment to delivering excellent customer service to residents, families, and staff
Requirements:
High school diploma required; LPN degree or diploma from an accredited program
1 year of experience as a Licensed Practical Nurse (LPN) in a skilled nursing facility or equivalent setting
Current LPN licensure in the state of New York
Ability to work varying shifts, including days, evenings, and weekends
Strong communication and interpersonal skills
Ability to maintain confidentiality and discretion in handling sensitive patient and facility information
