Position Overview
The MDS Coordinator is responsible for the accurate and timely completion of MDS assessments and care plans, ensuring compliance with all state and federal regulations. This role works closely with the interdisciplinary team, DON, and administration to support quality care, survey readiness, and reimbursement optimization.
Key Responsibilities
- Coordinate the entire MDS and Care Planning process
- Maintain care plan schedules and ensure documentation compliance
- Ensure timely electronic submission of MDS assessments
- Coordinate Interdisciplinary Care Plan Meetings
- Participate in Monthly and Quarterly QAA Committees
- Serve as Coordinator of the Wound & Skin Subcommittee
- Coordinate weekly Medicare Utilization Review meetings
- Perform Quality Indicator, PPS, and utilization analyses
- Collaborate closely with DON, Nursing Supervisors, and Administration
- Support staff education, mentoring, and professional development
- Promote positive communication among staff, residents, and families
- Serve as a leadership liaison between nursing staff and administration
- Assist with supervisory or staff RN duties as needed
- Attend required facility meetings
Qualifications
- Active Virginia RN license preferred
- Strong, experienced LPNs with extensive MDS/RAI experience will be considered
- At least 1 year of supervisory nursing experience in a hospital or extended care setting
- Prior MDS coordination or RAI process experience strongly preferred
- Strong clinical judgment and decision-making skills
- Organized, detail-oriented, and compliant with confidentiality standards
- Excellent communication and leadership abilities
- Compassionate and resident-focused approach to care
