Role Description The Clinical Pharmacist is a full-time, on-site role located in Watertown, WI, responsible for optimizing safe, effective, and evidence-based medication therapy for patients across inpatient and outpatient settings. Day-to-day responsibilities include verifying and dispensing medications, performing medication therapy management and medication reconciliation, and collaborating with physicians, nurses, and other clinicians to develop and adjust treatment plans. The role involves providing drug information and therapeutic recommendations, monitoring for drug interactions and adverse events, and participating in clinical rounds and interdisciplinary care conferences. The Clinical Pharmacist will also ensure regulatory and formulary compliance, support antimicrobial stewardship and other clinical initiatives, provide patient and staff education, and contribute to quality improvement and medication safety projects.
Qualifications
- Demonstrated proficiency in Pharmacy Practice and Pharmacy operations, including inpatient and outpatient workflows.
- Strong knowledge of Pharmaceutical Care and Medicine to support evidence-based therapeutic decision-making.
- Competence in Compounding and preparation of sterile and non-sterile medications, following applicable standards and regulations.
- Doctor of Pharmacy (PharmD) degree from an accredited institution; completion of PGY-1 residency strongly preferred.
- Current pharmacist license (or eligibility for licensure) in the state of Wisconsin, in good standing.
- Experience working in a hospital or health system environment, with familiarity in EMR systems and clinical decision support tools.
- Ability to collaborate effectively with interdisciplinary care teams, communicate clearly with patients and colleagues, and prioritize multiple clinical responsibilities.
- Commitment to patient-centered care, medication safety, continuous learning, and adherence to ethical and regulatory standards.
