Role Description
The Certified Diet Manager (CDM) will oversee dietary operations at The Edgewater, a 120 bed skilled nursing facility. Responsibilities include application of nutrition principles, accurate and timely documentation of nutrition information, communication with healthcare team and oversight of tray line.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Responsible for nutritional well-being of residents.
2. Conducts resident visitations and obtains food preferences and pertinent information as needed..
3. Conducts initial nutritional assessments as assigned.
4. Responsible for annual, quarterly and progress notes on assigned residents, including measurable, realistic goals for residents with nutrition concerns..
5. Responsible for accurate completion of Minimum Data Set, CAA's , and Care Plans for all residents
6. Attends and participates in care plan meetings as scheduled.
7. Monitors residents for changes in weight as necessary, and makes necessary interventions.
8. Participates / Facilitates weekly interdisciplinary meeting to discuss weights, documents same and follows up on recommendations
9. Communicates changes in resident nutritional status to physician and team with recommendations.
10. Updates and maintains current information on all care plans for all residents.
11. Counsels and educates residents on modified diets.
12. Conducts quality assurance checks.
13. Attends required meetings.
14. Provides in-service training/education.
15. Communicates with Dietitian regarding nutritional concerns of residents.
16. Works with speech, occupational therapy and restorative to provide interventions as needed.
17. Completes discharge summaries.
18. Conducts tray audits as scheduled weekly.
19. Conducts meal rounds as assigned weekly.
20. Attends departmental in-service meetings.
21. Supervises and works tray line as assigned.
22. Performs other duties as assigned by the Managing Director of Dining Services and/or the Operations Manager/Clinical Dietitian.
23. Maintains CEU hours for CDM credential.
Qualifications
High School diploma or G.E.D. Credentialed as a Certified Diet Manager (CDM) as certified by the Association of Nutrition and Food Service Professionals.
Long term care background is preferred.
Must be a minimum of 18 years of age.
Must be able to understand basic work instructions given in the English language.
Must be able to use evidence based practices.
Demonstrates initiative, organization, assertiveness, flexibility and the ability to get along with and work well with others.
Has the ability to implement and to interpret the programs, goals, objectives, policies, procedures, etc., of the department.
Must have patience, tact, cheerful disposition and enthusiasm as well as be willing to handle residents based on whatever maturity level they are currently functioning.
Must be willing to work beyond normal working hours, and in other positions when and where necessary.
Possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
Must be able to read, write and communicate effectively in English.
