Job Summary
Minimum Education - The Per Diem Nurse must be a registered nurse with an active license to practice in the State of Connecticut. The Per Diem Nurse may have one of the following: (1) B.S. in Nursing; (2) Associates Degree in nursing; (3) R.N. from approved diploma school of nursing.
Minimum Experience - One year experience in acute/sub-acute or home care setting.
Other Qualifications-
- Provides direct care and health teaching to community dwelling adults and adolescents in accordance with agency policies and procedures
- Communicates with Primary Care Nurse or Clinical Manager regarding patient status and needs
- Attends staff meetings, in-services
- Provides case management for patients when mutually agreed upon
- Performs all other duties that Management may determine necessary
- Reports to Clinical Manager

- The Per Diem Nurse must possess a valid driver's license and personal vehicle.
- Active Registered Nurse license in good standing in the State of Connecticut. 3) Provide proof of current certification in Cardiopulmonary Resuscitation from American Heart Association or American Red Cross.
- Physical, mental and sensory abilities sufficient to meet performance criteria with or without assistive devices.
