What are the responsibilities and job description for the Intake Nurse - VNA position at Waterbury Hospital?
JOB SUMMARY:
- Receives and processes referral intake information
- Demonstrates the ability to obtain and track Face to Face (F2F) documentation for applicable referrals
- Provides exemplary customer service to all referral sources
- Adheres to HIPAA Policies and Procedures
- Performs all other duties that Management may determine necessary
- Reports to Clinical Manager
QUALIFICATIONS:
Minimum Education and Experience
- LPN license in good standing in the State of Connecticut
- One year experience as an LPN in an acute/sub-acute or home care setting
- Proficiency in the use of computers and software applications
Other Qualifications:
- Physical, mental and sensory abilities sufficient to meet performance standards with or without assisted devices
- Effective oral and written communication skills
- Must have good organizational skills and be detail focused
- Ability to work in a fast paced environment with demonstrated ability to handle multiple competing tasks and demands
