Developmental Disabilities Case/Resource Manager - In-home Intensive Enhanced Case Manager.

The Home & Community Living Administration (HCLA) provides an advanced level of social services and specialized case and/or resource management for people with developmental disabilities and their families. The role adheres to federal and state laws and regulations, department/administration policies, and procedures governing work tasks. HCLA Region 2 office is inKent, WA.

Some Of What You Will Do

  • Identify the support needs of clients of the HCLA and plan for an authorized payment for appropriate services
  • Schedule and conduct face-to-face home visits with clients and families
  • Complete and revise client assessments and person-centered service plans within established timelines as required for caseload
  • Review and monitor client plans, reports, and client progress
  • Request Exceptions to Rule (ETRs) and prior approvals per requirements
  • Testify at administrative hearings as needed
  • Schedule and conduct at minimum quarterly home visits, including unannounced visits with clients and families/caregivers
  • While on home visits, observe living quarters, including the bedroom of the client, and document all visits
  • Based on assessed needs, develop person-centered service plans, and refer individuals to appropriate DDA services, other department services, and community programs/resources

We are looking for professionals who have

A Bachelor's degree in social sciences, social services, human services, behavioral sciences, or an allied field and two years of experience providing social services to people with developmental disabilities,

Or

Six years of experience in providing social services to people with intellectual or developmental disabilities,

Or

Satisfactory completion of 12 months as a Case Resource Manager Trainee,

Or

Equivalent experience/education.

Graduate training in social science, social services, human services, behavioral sciences, or an allied field will substitute, year for year, for one year of the experience providing social services to people with developmental disabilities.

  • Possess a high degree of communication skills (listening, observing, communicating, interviewing, understanding, confronting, engaging in problem-solving, and resolving conflict)
  • Compose professional, clear, grammatically correct correspondence
  • Organize the work environment and prioritize work assignments to complete timely and accurately
  • Listen, clarify, and follow directives, especially the ability to absorb new information and act on it
  • Possess basic computer skills, including the use of e-mail, and basic software, (e.g., word processing, etc) system applications (e.g., CARE and CMIS), and internet access
  • Demonstrate sensitivity to cultural diversity
