Position Summary
The role of the Stroke Program Coordinator reports to the Director of Nursing, Centers of Excellence. This position is responsible for coordinating patient care across the continuum as it relates to the stroke patient. This position collaborates with the members of the patient’s care team, the patient, and the patient’s family to ensure the goals of care are being met. This position partners with other departments as it relates to the stroke patient.
Essential Position Responsibilities
- Research data to provide ongoing collection of data pertinent to the program
- Develop and implement education and training materials for bedside nursing team on a continuous and ongoing basis
- Provide clinical knowledge and general understanding of the stroke patient as related to nursing care and provide education to staff, families, and patients to ensure care needs are met
- Communicate with department leaders and healthcare providers to ensure the care provided is consistent with evidence-based guidelines and meets the expectations of WRMC stroke program leadership
- Provide education to staff, families, and patients to ensure care needs are met
- Implement and deliver community education on an ongoing basis
- Perform data abstraction into national registry
- Function as program coordinator providing program management of data analysis, performance improvement plans, and educational materials
- Guide and assist with clinical research trials
- Facilitate the program with meeting Joint Commission and American Heart Association/American Stroke Association guidelines
- Collaborate with stroke providers to facilitate program growth
- Serve as a liaison for the stroke patient in aspects of the continuum of care
Qualifications
- Education: Must be a graduate of an accredited nursing program
- Licensure and Certifications: Registered Nurse license and CPR, required. Obtain a national certification within two years in either Advanced Stroke Coordinator or Stroke Certified Registered Nurse.
- Experience: Two years of nursing experience, required.
Work Environment: This position will spend up to 80% sitting while performing work in a standard office environment and 20% of the time standing and/or walking short distances while pushing, pulling, lifting and/or carrying up to 50 lbs.
