Bridge Program Service Coordinator
WCMHS Community Developmental Services (CDS) is seeking a compassionate Service Coordinator for the Bridge program. This role supports individuals with developmental disabilities and their families, coordinates person-centered services, and ensures care aligns with each person’s unique goals. You’ll supervise direct support staff, provide hands-on support when needed, and manage essential administrative tasks—all while collaborating with community partners to create meaningful, positive outcomes.

Summary of Duties:
- Coordinate person-centered services for individuals with developmental and intellectual disabilities and autism in home and community settings.
- Ensure services and care plans align with each person’s unique needs and goals.
- Supervise direct support staff and provide guidance on service delivery.
- Deliver some direct care as needed.
- Manage essential administrative tasks related to service coordination.
- Collaborate with team members and community partners to support positive outcomes.
- Apply creative problem-solving to address challenges in service delivery.
Compensation and Benefits
WCMHS provides a comprehensive benefits package including health, dental, vision, retirement, and a generous paid time off package including flexible holidays and 12 vacation days in year one.
The pay range for this position is $24.87-$25.12 per hour.
Summary of Qualifications
- High school completion or equivalent; BA preferred
- Three or more years’ experience providing services to individuals with developmental and intellectual disabilities and autism
- Knowledge of Behavioral Support Plans, Communication Plans, and Individual Support Agreements for individuals with intellectual and developmental disabilities and autism
- Ability to collaborate and work productively with internal and community partners
- Ability to document work/services provided into an electronic records system with a high level of accuracy and timely completion
- Team leadership experience strongly preferred
- Valid driver’s license, an acceptable driving record, and consistent, regular access to a safe, reliable, insured vehicle required
- Ability to provide services and complete treatment notes in English
- Working Conditions
- This position requires working in a number of environments including office spaces, program spaces, in the community, and in clients' living environments.
- Travel by car, including transportation of clients, is required.
- Work hours may include evening and weekend shifts as program needs require.
