Duties
- Conduct thorough patient assessments, including nutritional analysis and diagnostic evaluations to identify individual needs and health risks
- Develop, implement, and monitor personalized care plans focused on nutrition support, weight loss, chronic disease management, and long-term wellness goals
- Provide expert nutrition counseling utilizing motivational interviewing techniques to motivate behavioral change and foster client engagement
- Collaborate closely with healthcare professionals such as physicians, dietitians, and therapists to coordinate care and ensure integrated treatment approaches
- Utilize electronic health records (EHR/EHR systems) for accurate documentation of client interactions, progress notes, and care plan updates
- Educate clients on physiology, dietary needs, good sleep hygiene, exercise, and stress lifestyle factors that influence health.
Skills
- Proven experience in health coaching with a strong background in functional medicine, clinical nutrition and dietetics
- Supervising experience in healthcare or dietary settings preferred
- Extensive knowledge of physiology and chronic disease management strategies
- Skilled in developing care plans tailored to specific dietary needs including functional medicine dietary plans, acute care, weight loss, or long-term care scenarios
- Proficient in diagnostic evaluation methods and nutritional analysis to inform treatment decisions
- Expertise in motivational interviewing and nutrition counseling to facilitate positive behavioral changes
- Ability to collaborate effectively with healthcare professionals across disciplines using EMR/EHR systems for seamless communication

Pay: $18.00 - $22.00 per hour
