Responsibilities:
Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities
Monitor and report changes in resident condition to licensed nurses and physicians
Develop and implement individualized care plans in collaboration with the healthcare team
Participate in interdisciplinary team meetings to discuss resident progress and plan for ongoing care
Maintain accurate and detailed records of patient care and medication administration
Collaborate with therapy staff to ensure comprehensive rehabilitation services
Maintain a safe and clean environment for residents, staff, and visitors
Requirements:
Current LPN licensure in the state of Missouri
High school diploma or equivalent required; associate's or bachelor's degree preferred
0 years of experience in a skilled nursing or rehabilitation setting; recent graduates welcome to apply
Ability to work varying shifts, including evenings, weekends, and holidays
