We are seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our home health team on a PRN basis. The SLP will evaluate and treat patients in their homes to improve communication, cognition, swallowing, and overall functional independence.

PRN Home Health Speech Language Pathologist (SLP)
Position Title: PRN Home Health Speech Language Pathologist (SLP)

Position Summary
We are seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our home health team on a PRN basis. The SLP will evaluate and treat patients in their homes to improve communication, cognition, swallowing, and overall functional independence.

Responsibilities
- Perform comprehensive speech therapy evaluations and develop individualized treatment plans
- Provide skilled therapy services for speech, language, cognitive, voice, and swallowing disorders
- Assess and treat patients with dysphagia and provide dietary and safety recommendations as appropriate
- Educate patients and caregivers on communication strategies, swallowing precautions, cognitive support, and home exercise programs
- Monitor patient progress and adjust treatment plans as needed
- Complete accurate and timely clinical documentation in the electronic medical record
- Communicate effectively with physicians, nurses, therapists, and interdisciplinary team members
- Participate in care coordination and case conferences as needed
- Ensure compliance with Medicare, state, and agency regulations
- Travel to patient homes within assigned territory
- Master’s degree in Speech Language Pathology from an accredited program
- Current Speech Language Pathologist license in the state of Virginia
- Certificate of Clinical Competence (CCC-SLP) preferred
- Current CPR certification
- Home health experience preferred, but not required
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication, organizational, and clinical skills
- Ability to work independently and manage time effectively
