We are seeking a dedicated and compassionate Physical Therapist (PT) to join our home health team. The Physical Therapist will evaluate and treat patients in their homes, helping them improve mobility, strength, balance, endurance, and overall functional independence.

Home Health Physical Therapist (PT)
Position Title: Home Health Physical Therapist (PT)
Job Type: Full-Time / Part-Time / PRN

Position Summary
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our home health team. The Physical Therapist will evaluate and treat patients in their homes, helping them improve mobility, strength, balance, endurance, and overall functional independence.
This role offers flexibility, autonomy, and the opportunity to build meaningful relationships while delivering high-quality care in the home setting.
Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Provide skilled therapy interventions including therapeutic exercises, gait training, balance training, transfer training, and pain management
- Assess patient functional status and establish measurable goals
- Educate patients and caregivers on home exercise programs, mobility safety, fall prevention, and disease management
- Monitor patient progress and adjust treatment plans as appropriate
- Complete timely and accurate clinical documentation in the electronic medical record
- Coordinate care with physicians, nurses, therapists, and interdisciplinary team members
- Participate in case conferences and care planning meetings
- Ensure compliance with Medicare, state, and agency regulations
- Supervise Physical Therapist Assistants (PTAs) as applicable
- Travel to patient homes within assigned territory
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of Virginia
- Current CPR certification
- Home health experience preferred, but not required
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication, clinical, and organizational skills
- Ability to work independently and manage time effectively
