We are seeking a compassionate and skilled Occupational Therapist (OT) to join our home health team. The Occupational Therapist will evaluate and treat patients in their homes to help them regain independence, improve safety, and maximize their ability to perform daily activities.

Home Health Occupational Therapist (OT)
Position Title: Home Health Occupational Therapist (OT)
Job Type: Full-Time / Part-Time / PRN

Position Summary
We are seeking a compassionate and skilled Occupational Therapist (OT) to join our home health team. The Occupational Therapist will evaluate and treat patients in their homes to help them regain independence, improve safety, and maximize their ability to perform daily activities.

Responsibilities
- Evaluate patients and develop individualized occupational therapy treatment plans
- Provide skilled therapy interventions focused on improving activities of daily living (ADLs), strength, coordination, balance, cognition, and home safety
- Assess the need for adaptive equipment and environmental modifications
- Educate patients and caregivers on safety, energy conservation, fall prevention, and adaptive techniques
- Monitor patient progress and modify treatment plans as appropriate
- Complete timely and accurate clinical documentation in the electronic medical record
- Communicate effectively with physicians, nurses, therapists, and interdisciplinary team members
- Participate in care coordination and case conferences as needed
- Ensure compliance with Medicare, state, and agency regulations
- Supervise Certified Occupational Therapy Assistants (COTAs) as applicable
- Travel to patient homes within assigned territory
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in the state of Virginia
- Current CPR certification
- Home health experience preferred, but not required
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication, organizational, and clinical skills
- Ability to work independently and manage time effectively
