Home Health Aide (HHA)
Job Summary:
The home health aide provides personal care and related services in the home under the direction, instruction and supervision of the staff RN and the Nursing Director and/or appropriate supervisor.
Organizational Relationship: The HHA reports directly to the RN Supervisor
Risk Of Occupational Exposure To Blood Borne Pathogens: A: High Exposure
Qualifications:
1. Completion of a MD approved HHA course and competency and have received additional home care training (including communication skills, techniques of home client care, maintenance of clean, safe home environment, food & nutrition, infection control, caring for clients with special needs, caring for clients with terminal needs) and been skills competency tested.
2. Experience as a HHA for at least 1 year within the past 3 years.
3. Must have current CPR certification.
Responsibilities:
Tasks to be performed by a HHA must be assigned by and performed under the supervision of an RN who will be responsible for the client care provided by the HHA.
Duties of the HHA may include:
1. The provision of personal care services/assist with ADLs and iADLs as per the POC
2. To follow the POC for each client and document the same
3. Helps the client to maintain good personal hygiene
4. Assists in maintaining a healthful, safe environment
5. Plans and prepares nutritious meals. Markets when indicated on the POC
6. Assists the client with ambulation as per the POC
1. Assists with assigned treatments as ordered by the physician and approved/supervised by the nurse
2. May cue for medications ordinarily self-administered as assigned
3. May administer medications if certified as a certified medication technician
4. Taking and charting vital signs
5. Encourages the client to become as independent as possible according to the nursing care plan
6. Attempts to promote client’s mental alertness through involvement in activities
7. Gives simple emotional and psychological support to the client and other members of the household
8. Establishes a relationship with client and family which transmits trust and confidentiality
9. Prepares a report of her visit on the day it is performed and incorporates same in the clinical record weekly or as directed
10. Reports any change in the client’s status or home situation to her immediate supervisor, the staff nurse, or the aide supervisor
11. Carries out their assignment as instructed by the nurse or the paramedical team and documents and reports to the nurse when she is unable to do so
12. Works with personnel of other community agencies involved in the client’s care as directed by the staff nurse
13. Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse
14. Attends in-service as required by state and federal regulations
15. Confirming on a weekly basis, the scheduling of visits with the Supervisor/Director, to coordinate necessary visits with other personnel.
1. Notifying the RSA of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with the RSA.
2. Wears the RSA’s name badge at all time they are seeing RSA clients.
Functional Abilities:
Must be able to read 12 point or larger type.
Must be able to hear and effectively communicate in English.
Must be able to stoop and bend.
Must be able to travel to prospective clients’ residences.
Must be able to lift, turn, and transfer clients weighing up to 200 pounds
Must be able to carry bundles weighing up to 10 pounds upstairs.
