WSC provides this by offering quality, outcome-based services which meet evolving stakeholder's needs and bring together caring, engaged, and informed family members, friends, community partners, and employees to support these individuals.
WSC is actively seeking a Case Worker to provide supports for adults with developmental disabilities who are living independently or with their families. This position ensures that the individuals achieve/maintain independence and control of their lives. Our Case Worker will join our growing team to provide coverage in the Concord/Acton/Littleton (Central Middlesex County) area.
This role is interesting because you will:
- Balance a regular caseload dependent upon each individual’s hours of support
- Provide skill training to both individuals and outside staff, medical and benefits coordination, supportive counseling, and crisis intervention as directed by the individual.
- Promote activities and opportunities leading to personal growth and accomplishment.
- Develop a therapeutic relationship with individuals
- Assist with the development, implementation, evaluation and documentation of Individuals Treatment Plans (ISP, etc.).
- Supports and monitors individual funds as appropriate - provide guidance/support to ensure financial needs are addressed.
- Complete behavior modifying medication assessments
- Promote individual choice and self-expression based upon quality service principles. Ensure that individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information.
- Provide oversight of individual’s home environment to ensure compliance with basic safety practices.
- Provide transportation as appropriate in response to individuals’ schedules and desired activities and/or appointments. *Uses personal vehicle and public transportation.
You are qualified if you have:
- Associate's degree (preferred)
- Valid drivers license required - Use of personal vehicle
- Covid19 vaccination card
- Ability to pass background checks
- Ability to communicate effectively in English
- Ability to independently complete computer based training
