- Compensation: $125,000-$150,000 annually (DOE), Language differential available
- Newer graduates typically start closer to the lower end of the range; higher compensation reflects additional years of primary care experience
- Schedule: Full time, weekday primary care schedule
- Location: Onsite at our Rockwood Clinic: 18633 SE Stark Street, Portland, OR

- Collaborate closely with behavioral health, nursing, and care coordination teams
- Maintain timely, accurate documentation and population health follow‑up

Required
- Current, unrestricted Nurse Practitioner license in Oregon
- Family Nurse Practitioner (FNP) certification (role serves pediatrics through geriatrics)
- Current DEA and BLS
- Commitment to team‑based, patient‑centered primary care
- Experience in outpatient primary care, community health, or FQHC settings
- Comfort managing chronic conditions and preventive care across the lifespan
- Experience serving Medicaid or higher acuity populations
- Collaborative, organized, and patient‑focused clinical approach
