Specific Duties:
- Assessment and Planning:
- Participate in the development and implementation of Individual Program Plans (IPPs).
- Conduct regular assessments to monitor progress and adjust support plans as necessary.
- Advocate for the rights and needs of individuals, ensuring they receive appropriate services and support.
- Facilitate self-advocacy by teaching individuals how to express their needs and preferences effectively.
- Provide training in daily living skills, social skills, and employment-related skills.
- Use person-centered approaches to tailor training to each individual's abilities and goals.
- Health and Safety
- Administer medications and provide health care support as needed.
- Ensure a safe living environment by following health and safety protocols and conducting regular safety checks. Community Integration:
- Support individuals in participating in community activities and accessing community resources.
- Assist with transportation needs, ensuring individuals can attend appointments, work, and social activities.
- Maintain accurate and up-to-date records of services provided, incidents, and progress notes.
- Report any concerns or incidents promptly to supervisors and follow established reporting procedures.
- Work collaboratively with other staff, supervisors, and external agencies to provide comprehensive support.
- Participate in regular team meetings and training sessions to enhance skills and knowledge.
- Ability to lift and transfer individuals as needed.
- Willingness to work flexible hours, including evenings, weekends, and holidays, as required by the needs of the individuals.
Supported Living Service (SLS) Instructor Qualifications
- High school diploma/GED required, some college preferred
- Minimum of one to three years’ experience in the field of human services or supporting adults with developmental disabilities
- Able to read, write and converse fluently in Standard English and or Spanish
- All staff must undergo a physical examination and have a current TB test.
- Provide for the care and safety of persons without physical or verbal abuse, exploitation, or prejudice
- CPR First Aid Certified
- Skilled in Microsoft Office, Excel, and Outlook
- Excellent time management skills and ability to prioritize multiple tasks
- Strong problem-solving skills and attention to detail
- Excellent verbal and written communication skills
- Must possess current California driver’s license with insurance
