What are the responsibilities and job description for the Prescription Refill Nurse (3526) position at WACO FAMILY MEDICINE?
PRIMARY DUTIES:
- Accurately enter prescription refill requests data in Epic systems and route to prescribing clinician per protocols

Coordinate and communicate refill approvals or denials to pharmacies via phone or fax

Document and refill activities in the patient’s chart in Epic
-
Notify patients or caregivers when prescription status changes-approved, denied, called in, or ready for pickup

Process refills under established standing orders and empower in basket workflows
-
Follow refill SDOs for common chronic medication, and escalate when clinical criteria are not met
OTHER DUTIES:
- Conduct patient education regarding medication regimens and compliance
- Schedule follow-up or monitoring appointments when needed
- Mentor and train nursing staff in the refill workflows and Epic documentation practices
- Keep physicians updated on formulary or policy changes affecting IPP/Medicaid preferred medication coverage
- Support internal quality projects and improvement initiatives as assigned
- Assist in continuous workflow optimization and proposal of new refill protocols

PRESCRIPTION REFILL NURSE
REPORTS TO: Chief Nursing Officer/Chief Medical Officer
EDUCATION & EXPERIENCE:

Diploma or Degree in Nursing from an accredited nursing program.

Active LVN license in Texas
-
Current BLS certification
-
Prior experience in outpatient primary care or ambulatory refill management is strongly preferred.
SKILLS & COMPETENCIES:

Excellent written and verbal communication, including proficiency in medical terminology and medication names/use

Strong teaching and training abilities

Fluent in English; bilingual in Spanish preferred

Solid computer literacy: epic EMR, standard office software, phone systems

Detail-oriented with strong spelling and documentation accuracy

Ability to multitask and manage patient and provider interactions efficiently
