Wabash Senior Living & Rehabilitation is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house rehabilitation team. You'll work alongside an experienced interdisciplinary team dedicated to helping residents regain independence, improve mobility, and achieve their highest level of function.

Responsibilities

• Evaluate residents and develop individualized treatment plans.

• Provide skilled physical therapy services.

• Monitor resident progress and modify treatment plans as needed.

• Collaborate with physicians, nursing staff, therapy team, residents, and families.

• Complete required documentation accurately and timely.

• Ensure compliance with all state and federal regulations.

Qualifications

• Current Illinois Physical Therapist License.

• Strong communication and interpersonal skills.

• Skilled nursing or rehabilitation experience preferred, but new graduates are encouraged to apply.
