Now Hiring: Member Care Coordinator- Remote
Location: Raton, NM / Surrounding Area
Work Arrangement: Field-Based Remote
Travel: 25–50%
Schedule: Full-Time
We are seeking a Member Care Coordinator to support members with their healthcare needs through home health assessments, member outreach, education, care-plan follow-up, and coordination of healthcare and community resources.
What You’ll Do
- Conduct home health assessments and update member information.
- Contact and engage members regarding their healthcare plans and available benefits.
- Conduct follow-ups and check-ins on individual care-plan goals.
- Help members access healthcare services, appointments, transportation, and community resources.
- Identify member needs and barriers and escalate concerns to the appropriate clinical team.
- Work closely with the Medical Management/Case Management team to support member outcomes.
- Respond to member and provider inquiries and complete required documentation/data entry.
- Maintain accurate records while following HIPAA and compliance requirements.
What We’re Looking For
Candidates should have one of the following:
- Bachelor’s degree in Social Work or Psychology
- LVN/LPN 1 year of managed care experience
- RN
- 3 years of care coordination with a state-managed or waiver program
- 3 years of managed care experience
Additional experience with health insurance, member services, care coordination, health assessments, Medicaid/Medicare, or healthcare programs is highly relevant.
Required Skills

- Member education and motivational interviewing/coaching
- Microsoft Office/computer proficiency
- Strong documentation and organizational skills
- Valid driver’s license, reliable transportation, and applicable insurance
- Willingness to travel 25–50% for field visits
