Job Summary

This role supervises clinical staff, manages care delivery, and ensures effective collaboration across interdisciplinary teams to support optimal patient outcomes.
Key Responsibilities
- Review and assess patient referrals, including conducting home visits as needed
- Coordinate patient care plans and assign appropriate clinical staff
- Collaborate with physicians and interdisciplinary teams to determine eligibility and care needs
- Monitor and evaluate patient care through regular case reviews, home visits, and team conferences
- Provide clinical guidance, training, and support to staff to enhance performance and service delivery
- Lead case conferences to ensure effective communication and care coordination
- Conduct regular chart audits and provide feedback for quality improvement
- Participate in hiring, onboarding, and mentoring of clinical staff
- Support the development and delivery of training and continuing education programs
- Ensure compliance with regulatory standards, policies, and best practices
- Contribute to quality assurance and performance improvement initiatives
- Engage in community outreach and support program development efforts
- Perform additional duties as assigned
Education:
- Associate's degree in Nursing required; Bachelor's degree (BSN) preferred
- Graduate of an accredited nursing program
- Minimum of 2 years of supervisory or management experience in hospice, home health, or a related setting
- Experience working within interdisciplinary care teams
- Active, unrestricted Registered Nurse (RN) license
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong leadership and team management skills
- Excellent clinical assessment and decision-making abilities
- Effective communication and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
- Knowledge of healthcare regulations and compliance standards
- Strong organizational and documentation skills
- Commitment to quality patient care and continuous improvement
- Combination of office and fieldwork (including patient home visits)
- Requires travel within the assigned service area
Salary : $83,000 - $88,000
