Setting: Home Health

Pay Range: $2400-$2600/weekly

Schedule: 08:00 - 17:00 (Discuss with recruiter)

Contract Length: 13 weeks

Candidate Type: Local and traveler friendly

Requirements: 1 year of experience as a PT; Qualified applicants MUST have at least 1 year of experience in Home Health; Valid Physical Therapist license; Be willing to obtain Colorado or compact licensure

Responsibilities:

• Deliver one-on-one, high-touch care to patients in their homes
  
• Conduct skilled nursing visits and perform assessments
  
• Create and manage individualized care plans
  
• Collaborate with families and interdisciplinary providers to improve patient outcomes
