We’re currently looking for a Physical Therapist for a Home Health contract.
Job Details:
Job Title: CWHH-Physical Therapist

Pay Range: 2518.40 – 2644.32

Contract Length: 13 weeks

Requirements: Current State License as a Physical Therapist – Required; Proof of current CPR certification – Required; Current driver’s license – Required; Must have reliable transportation and insurance – Required; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting – Required; Previous home care experience – Preferred
Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness of care.
- Document, review, and complete a Plan of Care (POC), progress notes, and other clinical records; revise as needed based on ongoing assessment data.
- Plan and deliver treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and assessments.
- Instruct patients and families in transfer techniques and equipment use, ensuring safety and proper technique.
- Educate patients and families about therapeutic procedures tailored to their age, condition, and learning readiness.
- Supervise and coordinate patient care activities with interdisciplinary team members.
- Apply infection control protocols to prevent contamination and disease transmission.
- Accept clinical assignments aligned with education and competencies.
- Perform additional duties as assigned.
