Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
• Administer medications and injections accurately, including IV infusions and injections with aseptic technique, ensuring patient safety and comfort.
• Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while adhering to infection control protocols.
• Provide specialized treatments including Botulinum toxin injections for applicable conditions and assist with procedures like catheterizations or gastric lavage when necessary.
• Coordinate care with multidisciplinary teams using electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to ensure seamless documentation and communication.
• Educate patients and their families on disease management, medication administration, nutrition, and health coaching tailored to individual needs across geriatrics, pediatrics, or behavioral health populations.
• Monitor patients’ physiological responses during treatments such as ventilator management or dialysis procedures; respond promptly to acute pain or emergent situations in compliance with hospital standards.
• Support discharge planning and case management efforts by evaluating patient progress and coordinating follow-up services within the community setting.
• Ensure compliance with HIPAA regulations and maintain accurate medical records while performing specimen collection/processing or laboratory testing when required.
• Triage patient concerns via phone or in-person visits to prioritize urgent needs effectively within home & community care contexts.

Requirements

• Valid RN licensure in the state of practice with current CPR/BLS certification.
• Proven experience in diverse clinical settings such as hospital medicine, ICU, emergency room, or post-acute care environments; critical care experience is highly valued.
• Strong familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth; proficiency in ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
• Hands-on skills in IV insertion, phlebotomy, laboratory specimen handling, and aseptic techniques; experience administering injections is essential.
• Knowledge of advanced nursing procedures such as trauma medicine at Level I/II trauma centers or neonatal ventilator management preferred.
• Ability to perform comprehensive patient assessments involving anatomy knowledge and physiology understanding; experience working with people with disabilities or developmental challenges is a plus.
• Excellent communication skills for effective triage and patient education; experience working with children or elderly populations is desirable.
• Flexibility to travel within assigned service areas; valid driver’s license preferred but not mandatory.
• Additional certifications such as wound suturing, ostomy care, or esthetic laser treatment are advantageous but not required.
