Full Time NHTD/TBI Service Coordinator (Consistently Hiring due to Company Growth)
Looking to hire Service Coordinators in the New York City Region. Serving counties of: Brooklyn and Queens
VRD Group Corp is a Conflict Free Case Management Agency providing services to individuals enrolled in the TBI/ NHTD Waiver. The individuals who are enrolled in the TBI/NHTD Waiver have the needs that would qualify them to be in a Skilled Nursing Facility, but have chosen to receive the services necessary while residing in the community. Service Coordination is necessary to help those individuals navigate the system, access new services and assist with keeping the individuals safe within the community.
VRD Group Corp is based in Brooklyn, NY and is expanding across the state. VRD Group Corp is looking to hire a Service Coordinator to cover counties listed above. The Service Coordinator is responsible for meeting in person with each individual on the caseload at least quarterly, and maintaining contact with the participants/informal supports throughout the month, renewing plans as necessary, monitoring deadlines, maintaining contact and alerting the Waiver Director with concerns and issues, maintaining individual’s files, coordinating Team Meetings and interacting with professional agencies to advocate for participants and linking participants on the assigned caseload to services within their community to increase their independence.
Minimal Requirements for Service Coordination Position:
Must Possess a Bachelor’s Degree AND at minimum of one experience providing Case Management Service to individuals with disabilities and/or seniors. Experience must demonstrate linkage and referral of services to community/waiver based resources for individuals with disabilities. This experience must be clearly outlined within resume. Specifically looking for someone who is bilingual with Russian and has experience working with the NHTD/TBI wavier Program.
The ideal candidate has excellent organizational skills with a high attention to detail, strong verbal and writing skills and is computer literate. Candidates should have the ability to successfully multitask, be able to work independently, but also a supportive member of the team. Knowledge of Google Drive, Epaces, Social Service Organizations and being bilingual are highly desirable , but not required.
Candidates must have access to reliable transportation as this position requires the Service Coordinator to visit participants in the home and at various locations in the community. The applicant must reside in New York State with the ability to travel to around to the above listed counties/areas. Salary variable depending on experience.
