Patient Care Coordinator

The Patient Care Coordinator is the first point of contact for new referrals and the day-to-day link between patients, families, facilities, and our providers. You'll handle intake, verify eligibility, build provider schedules by geographic area, and make sure every visit is confirmed, documented, and followed through.

Key Responsibilities

• Receive and process referrals arriving by fax, email, and phone from hospitals, home health agencies, skilled nursing and assisted living facilities, insurers, and families.
• Complete first-pass insurance checks and coordinate with billing for full verification before scheduling
• Enter and maintain patient demographics, referral documents, and visit records in our EHR (Aprima)
• Build and maintain provider schedules organized by ZIP code and route, working toward same-week scheduling for new patients
• Place confirmation calls ahead of scheduled visits and keep appointment statuses current (confirmed, rescheduled, cancelled)
• Coordinate urgent or add-on visits with providers and billing
• Serve as a friendly, patient point of contact for patients, caregivers, and facility staff
• Track and follow up on outstanding items — missing documents, unreachable patients, repeat cancellations
• Support the disability-exam service line with scheduling and packet submission as needed
• Maintain strict HIPAA compliance in all communications and record keeping

Qualifications

• High school diploma or equivalent required; associate's degree or medical office certificate a plus
• 1 year in a medical office, home health, hospital, or care coordination role preferred
• Comfortable with EHR systems, scheduling software, and Microsoft Office/Google Workspace; Aprima experience a plus
• Working knowledge of insurance verification and medical terminology preferred
• Strong phone presence and clear written communication
• Highly organized, able to manage competing priorities and follow through without reminders
• Reliable, punctual, and able to work on-site in Farmington, MI

VPS of USA provides in-home and facility-based medical care to patients across Michigan, and performs consultative examinations under contract with the State of Michigan Disability Determination Services (DDS). Our DDS line currently covers internal medicine and speech therapy, and we're adding psychiatry.
