Job Summary:

The Pharmacist will perform a wide range of pharmacist procedures. Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.

Responsibilities

  • Counsel patients regarding appropriate use of medications.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use, dose, and medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items.
  • Compound medications as required.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs, asthma classes, diabetic, hypertension, and high cholesterol.
  • Provide medication information and medication therapy guidelines to the clinical staff as required.
  • Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
  • Provide clinical direction to beneficiaries and pharmacy technicians and provide input into job performance evaluations.
  • Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
  • Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Ensure that work areas are clean and safe according to applicable regulations.
  • Document significant medication interactions, and pharmacy interactions with Prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) standards.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary
  • Pharmacist will be present and available to lead and coordinate pharmacy workflow.
  • The pharmacist will perform all duties at an acceptable speed and accuracy standard as established and deemed appropriate by the work center supervisor.
  • Must demonstrate teamwork and self-initiation in accomplishing the pharmacy mission.

Requirements

  • Bachelors or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE)
  • A current valid, unrestricted license to practice Pharmacy in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the US Virgin Islands
  • A minimum of one year of experience in outpatient or community as a Pharmacist
  • Knowledge of computer operations and proficiency in use of basic word processing, data entry and automated medical records.
  • Proficient in operation of standard office equipment, such as personal computers, copiers, fax machines, and telephone systems.
  • Must have knowledge and experience, or demonstrate comprehension during training, with software and databases currently employed at the MTF (e.g. Microsoft Office, Access, Excel and PowerPoint; Composite Health Care System [CHCS], Armed Forces Health Longitudinal Technological Application [AHLTA]).
  • Must possess knowledge, skills and computer program literacy to collect and analyze data.
  • Knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]), and The Joint Commission (TJC), and the Clinical Practice Guidelines (CPGs).
  • Must demonstrate ability to communicate effectively both orally and in writing.
  • Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.
  • Must have knowledge and experience in Patient Advocacy, Patient Privacy, and Customer Relations.
  • Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
  • Familiarity with Department of Defense (DoD), Federal, State, non-profit healthcare systems and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is desired.
