General Function:
This full-time, hourly position is responsible for providing resources, referrals, and follow-up for individuals, in Native American communities throughout Washington State experiencing behavioral health issues; provide linkage to available Tribal and community services and resources that are appropriate to the caller’s needs. This position will also coordinate with emergency department, hospital, and treatment facility staff to provide follow-up services for patients who have been discharged home. This position will provide follow-up and case management services to ensure individuals at risk are connected to services in their Tribal communities.
Principal Activities:
Engage with individuals via phone or Telehealth to assess needs and collaboratively develop plans for connecting them to appropriate services.
Provide excellent customer service and maintain accurate, professional documentation of all client interactions. Process referrals and coordinate with tribal and community partners to ensure seamless linkage to services, maintaining an up-to-date resource database for all counties in Washington State.
Build and sustain relationships with facility staff (e.g., treatment centers, hospitals, emergency rooms) to support follow-up care and post-discharge appointments for individuals
Understand and facilitate referrals to Mobile Crisis Outreach Teams across Washington State.
Support program functions through additional duties, including participation in community events like health fairs and tabling opportunities.
Contribute to an environment that focuses on ensuring integrity, respect, accountability, and trauma informed service

Requirements:
- Knowledge of Tribal communities and the Indian Health System
- Agency Affiliated Counselor Registration is required within 30 days of employment, or equivalent, and must remain in active status during employment
- Passing a typing test with a score of 35 WPM or higher
- Demonstrated respect for diversity

Home Office Technology Requirements
- High-Speed Internet: Minimum download speed of 100Mbps and upload speed of 10Mbps. To ensure smooth workflow, you will be required to provide a recent internet speed test result as proof
- Secured Wired Connection: A direct wired connection to the internet using a secured ethernet cable is mandatory. Wireless connections (WiFi) are not allowed due to potential security risks and performance limitations
- No Satellite Internet: Satellite internet service providers such as HughesNet, Starlink, or Visat are not supported due to latency issues that can negatively impact service delivery
