Function:
The Community Living Manager is responsible for providing leadership, oversight, and support to staff delivering Personal Care services in accordance with ADMH regulations, supporting staff in proper implementation of care plans, ensuring safe and compliant service delivery, coordinating transportation needs, ensuring accurate and timely documentation in the electronic billing system and responding promptly to emergencies. Reporting directly to the Coordinator of Community Integration Services, the Community Living Manager ensures that personal care supports are delivered safely, respectfully, and in full compliance with ADMH standards and agency policies.
Know How:
The Community Living Manager must be at least twenty-one (21) years of age and possess a high school diploma or GED, and a valid driver’s license. While not required, supervisory or leadership training is preferred. The ideal candidate will have experience supporting adults with developmental disabilities and demonstrate strong leadership, communication, and organizational skills. Effective communication with staff, individuals served, families, and interdisciplinary team members is critical.
Principal Activities:
- Implement ISPs, personal care plans, and behavior support/psychotropic medication plans under supervision, providing hands-on personal care, support with activities of daily living, and community integration.
- Complete all required documentation in accordance with ADMH, HCBS, and agency standards, and report updates, concerns, and observations to the Coordinator of Community Integration Services.
- Serves as a primary point of contact between individuals receiving services, VOA Southeast, families/guardians, support coordinators, healthcare providers, and community partners, ensuring that personal care and community integration supports are coordinated, high-quality, and person-centered.
- Assists with recruiting, screening, and selecting qualified personal care and community living staff committed to providing respectful, individualized care.
- Ensures all staff complete required ADMH and HCBS-mandated training and ongoing learning related to personal care services, dignity of risk, community inclusion, and safety.
- Coordinates and participates in interdisciplinary team meetings to ensure consistent communication, effective service planning, and compliance with person-centered planning standards
- Monitors the delivery of personal care supports, community inclusion activities, and implementation of all support plans to ensure services promote independence, safety, and meaningful participation.
- Assist with healthcare coordination to ensure individuals receive preventative care, follow-up medical treatment, and health supports in accordance with local, state, and federal guidelines.
- Maintains accurate, timely, and compliant documentation and oversees staff documentation in the electronic billing and record systems in alignment with HCBS and ADMH standards.
- Ensures program operations meet ADMH Minimum Standards for Personal Care Services, HCBS Waiver requirements, Medicaid regulations, and VOA Southeast policies and procedures.
