Qualifications

Certified Peer Support Specialist

Personal lived experience with mental health recovery and ability to appropriately share that experience.

Bachelor’s degree in social work, psychology, human services, or related field preferred (or equivalent combination of experience and education).

Prior Experience In Behavioral Health Case Management Strongly Preferred.

Strong organizational, advocacy, and communication skills.

Ability to work effectively in a collaborative, multidisciplinary, trauma-informed team environment.

Salary Range

$18-$20 per hour depending on experience

Job Summary

The EpiCenter Program is seeking a dedicated Peer Support Specialist / Case Manager to join our team. This unique hybrid role combines the lived-experience expertise of a Peer Support Specialist with the structured responsibilities of a Case Manager. The position is designed to foster a recovery-oriented, person-centered, and trauma-informed environment that empowers individuals to achieve their goals and thrive in the community.

The Peer Support Specialist / Case Manager will support clients who qualify for the EpiCenter program through outreach, advocacy, and individualized care coordination. This includes helping clients access natural supports, community resources, and treatment services while also modeling hope, recovery, and resilience through shared lived experience.

Job Duties

Provide peer-to-peer support based on shared lived experience of recovery.

Model wellness and resiliency strategies to promote empowerment and self-advocacy.

Build trusting, recovery-focused relationships that encourage engagement in treatment.

Facilitate connections to natural supports and community resources.

Use personal recovery journey to reduce stigma, foster hope, and inspire clients.

Assist clients in developing individualized recovery and treatment goals.

Coordinate care between clients, families, natural supports, providers, and stakeholders.

Identify and help remove barriers to accessing services and supports.

Link clients with community resources such as housing, employment, healthcare, and social services.

Monitor progress toward goals and document services in accordance with program and regulatory standards.

About The Role

This position is designed for a professional who can bridge two critical functions: offering peer support grounded in lived experience while also providing case management that ensures clients have access to comprehensive care and resources. By combining these perspectives, the Peer Support Specialist / Case Manager plays a central role in supporting recovery, improving quality of life, and fostering community integration for individuals served by the EpiCenter Program.
