As a Clinical Supports Coordinator Assistant, you'll help connect individuals to behavioral health services, healthcare, housing, transportation, employment resources, and other community supports while gaining valuable hands-on experience and professional development.
What You'll Do:
>> Support care coordination and service navigation
>> Assist with referrals, outreach, and appointment scheduling
>> Help individuals overcome barriers to accessing services
>> Support person-centered planning and service implementation
>> Maintain documentation in electronic health record systems
>> Work alongside experienced clinical and care coordination professionals
Qualifications:
>> Bachelor's degree in Social Work, Psychology, Human Services, Public Health, Counseling, Sociology, Criminal Justice, or a related field
>> No prior Supports Coordination experience required
>> Internship, volunteer, healthcare, social services, direct care, or community-based experience is a plus
