Minimum Qualifications:
- Bachelor’s degree or 60 college credits plus two years relevant experience developing community linkages.
- Requires a demonstrated ability in verbal and written communication, including basic computer skills.
- Manage a small caseload.
- Manage the resident intake and assessment process to orient the resident to the program and produce a comprehensive assessment of resident service needs.
- Develop with the resident a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the resident as required.
- Engage outside providers to develop quality linkages to support the needs of the residents.
- Develop with the resident a mutually agreed-upon wellness plan reflecting assessment findings. Update the plan with the residents, as necessary.
- Meet regularly with the resident to assist him/her in obtaining needed services/benefits to achieve their individual service needs. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
- Document counseling services and linkage services provided in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
- Design, conduct and coordinate staff trainings and presentations
- Provide opportunities for corporations and community groups to support program services. Design, conduct and coordinate resident presentations and groups
