- The SSVF Healthcare Navigator connects Participants to the VA for Health Care or Community Health Care Services.
- Bachelor’s Degree preferred or High School Diploma and 4 years’ experience.
- Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties.

Hourly payrate $18.20
