- The Case Manager in the role of Independent Living Coach is responsible for:
- Conducting psychosocial assessments and determining eligibility for services.
- Developing and implementing individualized Treatment/Life Plans.
- Providing ongoing supportive counseling to prevent relapse, psychiatric hospitalization, and incarceration.
- Facilitating participant intake, program development, referrals, and discharge processes.
- Offering coaching and counseling in areas such as personal and social adjustment, work training, and productivity.
- Maintaining detailed case records and adhering to all documentation requirements.
- Monitoring participants’ progress and participating in case review meetings with stakeholders.
- Positions will be in the office and/or onsite with participants. Virtual work is not available.
- Knowledge of community resources; experience with Continuum of Care and/or Veterans Affairs is a plus.
- Basic computer skills; fluency in English; bilingual in Spanish preferred.
- Valid Florida driver’s license with a clean driving record for use of company vehicles.
Qualifications:
- Bachelor’s Degree in a relevant field from an accredited institution.
- 2 years of experience managing cases involving mental health conditions, substance abuse, homelessness, or veterans.
- Knowledge of community resources; experience with Continuum of Care and/or Veterans Affairs is a plus
- Basic computer skills; fluency in English; bilingual in Spanish preferred.
- Pass Level II Background check and drug screening

Hourly pay rate $21.34 to $22.08 based on level of education
