Reporting to the Program Director, this role provides direct clinical social work services to clients in transitional/emergency housing and medical respite. Services include clinical assessment, consultation, therapy, and client-centered advocacy—helping clients stabilize, access resources, and move forward.

Services include clinical assessment, consultation, therapy, and client-centered advocacy—helping clients stabilize, access resources, and move forward.
What You’ll Do
- Provide direct clinical social work services, including assessment, consultation, therapy, and client-centered advocacy
- Interview clients, families, and involved agencies to gather social/clinical data for diagnosis and treatment planning
- Conduct assessments and develop plans that enhance social functioning and support community reintegration
- Engage clients therapeutically to address mental health symptoms and coping challenges in the social environment
- Maintain an up-to-date directory of resources (health care, social services, housing, entitlements/disability, vocational supports)
- Make and coordinate appropriate referrals/linkages, updating resources monthly or as needed
- Engage with referral source representatives as requested to support continuity of care
- Prepare reports with findings, conclusions, and recommendations as required by funding sources
- Document employment/treatment progress in permanent client files and collect required data
- Participate in required internal/external meetings, trainings, supervision, and staff conferences
- Maintain strict confidentiality and comply with administrative policies and procedures
- Complete other duties as assigned
- New Jersey Licensed Clinical Social Worker (LCSW) with 2 years of appropriate experience
- Working knowledge of Social Learning Theory, Cognitive-Behavioral interventions, and clinical assessment/evaluation
- Excellent written and verbal communication skills
- Outstanding organizational skills with the ability to multi-task and manage priorities
- Valid driver’s license and ability to meet agency driving standards
- Ability to pass a criminal background check
- Ability to handle Protected Health Information and comply with HIPAA Privacy and Security Rules
- Bilingual abilities are a plus
