What You’ll Do
- Conduct street outreach and attend community events to engage with at-risk individuals
- Provide emergency support, referrals, and transportation assistance
- Perform intake assessments and create individualized service plans
- Mentor clients and assist with accessing basic needs and ongoing care
- Maintain real-time documentation and ensure continuous support
- Facilitate group sessions and collaborate with community resources
- High school diploma or equivalent
- 2 years of experience working with homeless or behavioral health populations
- Compassion, strong communication skills, and the ability to problem-solve on the spot
- A valid driver's license and ability to meet agency driving standards
