The Opportunity
Reporting to the Patient Services Manager, the primary purpose of this position is to provide comprehensive and coordinated interventions to patients addressing the social determinants of health. A Community Health Worker (CHW) at Volunteers in Medicine is instrumental in supporting adults, children, and families in connecting with and obtaining community resources. In addition, CHW assists patients in navigating unfamiliar systems, identifies gaps in services, and advocates to create bridges, eliminate access barriers that promote health equity, systems change and a community where immigrants can thrive. The position is full-time, exempt, and paid with an annual salary and benefits. Because it is part of a small department in a dynamic organization that is highly visible in the community, the person in this position has great potential for success, growth, and professional fulfillment.
Responsibilities
· Assist patients and prospective patients in accessing available insurance programs
· Assess patients and families' strengths and needs and assist them by creating a care plan to access comprehensive and coordinated interventions according to their needs and priorities
· Document patient encounters and follow-ups in medical records and prepare reports
· Serve as a liaison between Volunteers in Medicine, the families, and the
organizations providing services
· Identify gaps in services in the community and support advocacy efforts and initiatives for systems change
· Participate in community-based activities including community events, meetings and committees in Berkshire County
· Develop and provide programs and services in collaboration with other agencies
· Assist the Patient Services Manager with specific projects to support care management efforts such as developing referral and educational resources for systems navigation, registration of patients and families for events, groups or programs, updating data bases, among other tasks
· Outreach to potential new patients and connect them to clinical and social services particularly focusing on the most vulnerable patient populations
· Respond to medical phone calls as needed when no medical staff is
available
· Respond to incoming Spanish/Portuguese language phone calls
· Occasionally provide interpreter support for ViM services
· Participate in population-based projects that enhance retention in care, self-management, treatment adherence, and compliance with accepted health care standards of care

Skills
An ideal candidate will be:
· A social worker with a US or a foreign degree; a student of social work; a professional in human services related careers; or a member of the
· community who has completed training in Community Health Work or who has experience working with immigrants.
· A person who has exposure and awareness of the cultural differences, challenges, and opportunities of working with immigrants or underserved population.
· Bilingual (a must!) with speaking, reading and writing skills in English/Spanish or English/Portuguese
· Attentive to details, time management and problem-solving skills.
· A team player who enjoys working with others.
· Skilled in Word, Excel and familiar with the use of databases.
· Must have authorization to work in the US
Pay: $47,000.00 - $50,000.00 per year
