Overview
The Hospice CNA provides personal care and support to people living with serious, life-limiting illness. Working under the direction of VHOCC nurses, the CNA helps patients stay safe, comfortable, and cared for in the place they call home. The area of service is from Diamond Point to Joyce.
Responsibilities
- Provide bathing, personal care, and help with other nurse-delegated tasks.
- Help patients move, walk, and transfer safely.
- Follow each patient's care plan and work closely with the nursing team.
- Schedule visits and communicate with patients, families, and caregivers.
- Watch for changes in a patient's needs or condition and report concerns to the nursing team.
- Educate patients and caregivers on safe personal care techniques.
- Make sure needed care supplies are available.
- Complete clear and timely documentation in the electronic medical record system.
- Attend team meetings and take part in VHOCC activities as needed.
- Cross-train to help with clinical administrative tasks when needed.
- Follow VHOCC policies and procedures and support the team as needed.

- Valid driver's license, reliable transportation, and ability to travel throughout the service area.
- Ability to use basic technology needed for the job.
Pay: $25.00 - $27.00 per hour
Expected hours: 20.0 – 25.0 per week
