Role Description This is a full-time hybrid Registered Nurse role based in Jackson, TN, with some work-from-home flexibility. The Registered Nurse will provide skilled nursing care to patients in their homes, including assessment, care planning, wound care, medication management, and monitoring of symptoms and vital signs. The role includes coordinating with physicians, caregivers, and an interdisciplinary team to develop, implement, and adjust individualized care plans. The nurse will educate patients and families on disease processes, medications, home safety, and self-care strategies to support independence and prevent hospitalizations. Daily responsibilities also involve documenting clinical visits, maintaining regulatory compliance, participating in case conferences, and contributing to quality and safety initiatives. Some administrative and care-coordination tasks may be performed remotely, while patient visits and in-person team meetings occur in the local service area.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of Tennessee; current BLS/CPR certification.
- Proven clinical assessment, critical thinking, and care-planning skills in home health, acute care, or a related setting.
- Ability to provide skilled nursing interventions such as wound care, medication management, and chronic disease management.
- Strong patient and family education skills, with the ability to communicate clearly, respectfully, and compassionately.
- Effective time management and organizational skills, including managing a home-visit schedule and documentation deadlines.
- Comfort with electronic health records and basic technology needed for remote charting and communication.
- Valid driver’s license, reliable transportation, and willingness to travel locally for home visits.
- Ability to work independently in the field while collaborating closely with an interdisciplinary team.
- Experience in home health or community-based care is preferred; familiarity with Medicare/Medicaid regulations is a plus.
